Tri-Homecare

Tri-Homecare is a homemaker/attendant care agency for Union County. Homemaker services may include but are not limited to: errands, cooking, cleaning and laundry. Attendant care services provide more personal care such as bathing and dressing.

Tri-Homecare’s primary goal is to provide services in the home that will enable people to remain independent as long as possible versus being placed in long term care facilities.


Lifestream Services

Applicants for in-home services are assigned a professional case manager who works on behalf of the clients, family and caregiver to secure and arrange necessary services. Eligibility for in-home services is based on need and physical impairments.

Lifestream contracts with Tri-Homecare to provide attendant care and homemaker services.

Attendant Care

Personal, hands on care. Assists with bathing, dressing, grooming and ambulation. Maintains a safe living environment.

Homemaker service

Assists in maintaining a clean, safe and healthy home environment. Also may include, running errands, grocery shopping, laundry and meal preparation.


Funding

Tri-Homecare provides services for the following funding programs: OPTIONS (CHOICE), Medicaid Waiver, and private pay.

To qualify for funded programs you must be assessed by a Lifestream case manager. If you do not qualify for Lifestream funding, you may pay for Tri-Homecare services by private pay.

Medicaid Waiver

The Medicaid Waiver Program is a Medicaid funded program that utilizes home and community based services. The applicant must qualify for institutional care. Placement is waived for home care. Lifestream referral required. You may make an appointment to have a Lifestream case manager visit you by calling 1-765-759-1121.

Private Pay

Tri-Homecare attendant care and homemaker services are also available for private pay clients. For current rate information please feel free to contact us. Private pay services are available to anyone.
